Waiver and Release of Liability

I hereby agree that I am voluntarily participating in a mosaic workshop, class, demonstration, or event which may include use of equipment, machines, tools, and/or materials that have been provided by The Mosaic Workshop, guest instructors, and/or persons who have helped organize the event (referred to hereafter collectively as “TMW”). I am at least eighteen (18) years of age.

Participants are responsible for their own safety. TMW recommends students wear appropriate eye and respiratory protection when using tools and equipment.

I agree that I am solely responsible for my own safety while at any TMW operated or affiliated facility. I hereby release and forever discharge TMW from any and all claims, complaints, liabilities, obligations, promises, damages, costs, or losses which may arise out of my participation in the aforementioned activities, including but not limited to claims for delays, cancellations, injuries, death, or loss of or damage to my person or property occurring during any portion of, or relating to, any of the workshops, classes, demonstrations, events, or studio usage described herewith. I agree that this release shall bind my heirs, executors, other legal representatives, and members of my family.

I authorize TMW to arrange for emergency medical care in the event of an emergency, and I agree that I shall be solely responsible for all costs and expenses of such medical treatment.

TMW assumes no responsibility for losses and expenses caused by illness, weather, wars, or acts of nature. TMW reserves the right to make changes to the workshop, class, or schedule at any time.

I understand that images, photos or videos may be taken by TMW or other participants that may contain images of me in part or in whole. I agree that these images may be reproduced by TMW and I hereby grant TMW a perpetual, royalty-free, worldwide, irrevocable license to reproduce such images, photos or video in any medium for promotion and publicity purposes.
